Important Information Regarding Your Initial Consultation
All new patients must be referred by a GP or another Specialist
New patients will be offered a 40min initial appointment
All patients will be seen by the Specialist; however, you may be assessed by a Trainee Specialist for first part of the consultation
If pre-appointment tests are requested, please ensure they are completed 3-5 working days before your appointment
A carer or support person is welcomed
Interpreters can be arranged on request
Telehealth appointments are also provided but in-person visits are preferred for initial consultations
Preparing for your consultation
• Gather personal health information: home blood pressure and blood sugar records, home dialysis records, medication information and recent lab test results
• Measure and record personal observations: weight, pulse rate, blood pressure and any other measurements you take regularly e.g., blood sugar levels
• Make a list of the things you want the doctor to address including questions, repeat prescriptions

Medication details: a current list, photos on your phone or the actual medications themselves
Medicare card
Any other relevant health information
After your consultation
Fill any new prescriptions and start taking them as directed
Arrange any follow-up investigations before your next appointment
The specialist will write a letter back to your GP and you will receive a copy
